BY CLICKING "ACCEPT" AT THE END OF THIS DOCUMENT AND/OR BY THE USE OF FAMILY HORIZONS CREDIT UNION'S HOME BANKING SERVICE (THE "SERVICE") I AM CONSENTING (INITIALLY AND ON AN ONGOING BASIS) TO: 1) THE ELECTRONIC DELIVERY TO ME BY YOU OF THIS DISCLOSURE AND AGREEMENT, THE ELECTRONIC DELIVERY OF MY PERIODIC STATEMENTS AND ANY OTHER SUBSEQUENT DISCLOSURE OF INFORMATION RELATED TO THIS SERVICE INCLUDING ANY CHANGE IN TERMS NOTICE; AND 2) THE PROVISION TO ME, ON AN ONGOING BASIS, OF FAMILY HORIZONS CREDIT UNION'S HOME BANKING AND BILL PAYMENT SERVICES (COLLECTIVELY, THE "SERVICE") SUBJECT TO THE TERMS AND CONDITIONS OF THIS DISCLOSURE AND AGREEMENT.

TO OBTAIN A PAPER COPY OF THE DISCLOSURES, I MAY WRITE TO FAMILY HORIZONS CREDIT UNION AT 6665 E. 21st Street, Indianapolis, IN 46219 WITH THE DETAILS OF MY REQUEST. I UNDERSTAND THAT IF I REQUEST PAPER COPIES, THEY WILL BE PROVIDED TO ME AT NO CHARGE.

WITHDRAWAL OF CONSENT. I UNDERSTAND THAT I MAY WITHDRAW SUCH CONSENT AT ANY TIME BY NOTIFYING YOU AT: FHCU@FAMILYHORIZONS.COM OR 6665 E. 21st Street, Indianapolis, IN 46219. THE SERVICE WILL BE TERMINATED WITHIN A REASONABLE TIME THEREAFTER WHEN YOU HAVE HAD AN OPPORTUNITY TO ACT ON MY INSTRUCTIONS.

Minimum System Requirements: In order to receive and review disclosures electronically and conduct electronic transactions through the ("Service"), I must have access to a computer with Internet access and Adobe Acrobat Reader installed in order to view and/or print my monthly and/or quarterly statements.

In addition, I have and will maintain a printer capable of printing any disclosures that are e-mailed to me and/or made available on your Website. In the alternative, I have and will maintain the ability to electronically save and visually display on my computer screen any disclosures that are e-mailed to me and/or made available on your Website. I understand that you do not make any warranties on equipment, hardware, software, Internet service provider, or any part of them, expressed or implied, including, without limitation, any warranties of merchantability or fitness for a particular purpose.

Change of E-Mail/Postal Address: I must promptly notify you of any change in my e-mail or postal address.

Copy Received. I acknowledge receipt of a copy of this Disclosure and Agreement electronically. You recommend that I print and retain a copy of this Disclosure and Agreement for my records. I may obtain additional copies of this Disclosure and Agreement by accessing the Service and clicking on the EFT Disclosure link located on the login page.
